The assay is based on the enzymatic hydrolysis of p-nitrophenyl phosphate (PNPP).

Under alkaline conditions:

PNPP + H₂O → p-Nitrophenol + Phosphate

This reaction is catalysed by alkaline phosphatase present in the patient sample.

The rate at which p-nitrophenol forms is measured by the analyser as an increase in absorbance. The rate of absorbance change is proportional to the ALP activity in the serum specimen.
